Rhinoplasty

Rhinoplasty is a procedure to reshape the nose, and can be coupled with internal procedures to improve nasal breathing. It is usually performed as an outpatient surgery, and normally takes two to three hours. Most incisions are made on the inside of the nose, but some require a small incision that is hidden at the base of the nose. This scar heals very well and is usually not noticeable even at close range. The procedure is determined by the type of correction needed.

Dr. Bull prefers general anesthesia for rhinoplasty but on the less complicated procedures, a local anesthetic with sedation may be all that is needed.

The Surgery

During rhinoplasty surgery, Dr. Bull separates the skin of the nose from the supporting framework of bone and cartilage. He then reshapes the framework and redrapes the skin. In most cases involving a reduction in size or shape of the nose or the removal of a hump, incisions are made inside the nose. A narrower nasal bridge is formed moving the nasal bones closer. If a reduction in the nasal tip is desired, the cartilage is resized through the inside incision, but may also require a small incision at the base of the nose. The angle between the nose and the upper lip can be improved by elevating and trimming the septum, the dividing wall between the two chambers of the nose. In some cases, it is necessary to narrow the base of the nose. This procedure involves removal of skin from both sides of the nostrils at the base.

A lightweight splint is applied to maintain the new shape of the nose. The splint is usually removed within a week. Nasal pads may be inserted at the time of surgery to protect the septum.

This packing is removed within two to three days. Before and even after the packing is removed, there is frequently some stuffiness of the nose but this resolves with time.Pain connected with the surgery is minimal to moderate and is controlled with oral medication. Bruising around the eyes occurs but begins to fade within a few days. Discoloration usually disappears in two to three weeks.

Headaches and nosebleeds may also occur but will improve quickly. Swelling can be reduced by keeping the head in a slightly elevated position when reclining. Cold compresses to the cheeks may also help. Some slight swelling may be present for as long as four months to a year.

Risks include infection, bleeding, scarring, and sensory changes. There are other risks dependent on the type of specific procedure performed that should be discussed with Dr. Bull preoperatively. Some patients have incomplete resolution of the problem that may require further surgery, but this occurs in less than five percent of patients.

Getting Back to Normal After Nasal Surgery

Although most rhinoplasty Chicago patients are usually up and around the day of the procedure, strenuous exercises, particularly those which might elevate blood pressure, should be avoided for at least 4 weeks. Patients should avoid sun exposure for about eight weeks. The nose takes some time to settle into its new shape, and the final appearance may not be achieved for a year.Rhinoplasty performed to correct congenital deformities, relieve breathing problems or repair defects caused by trauma may be covered by insurance. Cases are judged on an individual basis and require pre-authorization.
